Acknowledgement and consent for home medical care

1. Consent to the home visit

I, the user/patient, acknowledge that I freely agree to receive medical care at my home or at the address I have given, from a doctor or nurse of Tabassem Plus Medical Center, and I understand that this service is provided within the limits of what is possible outside the center.

2. Nature and limits of the service

1. I understand that a home visit is for services that can be provided safely at home, such as follow-up care, dressing changes, injections, vital-sign checks and monitoring after a procedure.
2. I understand that the equipment and tests available at home are limited compared with the center, and that the doctor or nurse may recommend a visit to the center or further tests to complete the diagnosis or treatment.
3. I acknowledge that a home visit is not an emergency service and does not replace emergency care, and that in an emergency I must call the Red Crescent on 997 or go to the nearest emergency department.
4. I understand that the doctor or nurse may decline or stop the service if they judge it unsafe at home, or if my condition requires transfer to the center or to a hospital.
5. I undertake to provide accurate and complete information about my health, medical history, the medicines I take and any drug allergies, and to tell the medical team of any change in my condition before the visit.

3. Address and visit time

1. I confirm that the address on my account is correct and complete, and I am responsible for updating it before the visit if it changes.
2. I undertake that I, or someone acting for me, will be at the address at the booked time, and that the mobile number on my account can be reached.
3. I understand that the arrival time may be slightly delayed by traffic or urgent cases, and that the team will contact me if there is a significant delay.
4. I understand that if no one is at the address, or it cannot be reached, the visit may be treated as a no-show under the center's policy.

4. The visit setting and the safety of the medical team

1. I undertake to provide a suitable, clean and well-lit place for the care, preserving the patient's privacy.
2. If the patient is a minor or unable to make decisions, I undertake that their guardian or legal representative will be present throughout the visit.
3. I undertake to keep pets away during the visit and to provide a safe and respectful setting for the medical team, and I understand that the team may end the visit if it faces any abuse or danger.

5. Privacy and personal data protection

1. I agree to the collection and processing of my personal and health data and my address, as needed to provide and document the home visit and to manage the medical service, in line with the regulations in force in the Kingdom of Saudi Arabia.
2. I understand that the medical team will record notes of the visit in my medical file at the center, and that my data is treated as confidential and accessible only to those authorized.

6. Responsibility and liability

1. I alone am responsible for the accuracy and completeness of the health information, address and contact details I provide, and I acknowledge that the center and its team are not responsible for any outcome resulting from incorrect, incomplete or withheld information provided by me or on my behalf.
2. I acknowledge that home care is provided with the means available outside the center, and that, to the extent permitted by law, the center is not responsible for any limitation arising from the restricted equipment and tests at home, which I accepted in choosing this service.
3. I am responsible for following the medical instructions and recommendations, including attending the center or an emergency department when directed, and I acknowledge that the center is not responsible for any harm resulting from my failure to follow them.
4. I am responsible for the safety of the place where the visit is given, and I acknowledge that the center is not responsible for any injury or damage arising from the condition of the home, its contents, pets or the people present.
5. I am responsible for keeping valuables and money safe in the home during the visit, and I acknowledge that the center is not responsible for their loss or damage.
6. I acknowledge that the center is not responsible for delays or failure to arrive caused by roads, weather or anything beyond its reasonable control, and that in such a case the visit will be rescheduled.
7. In all cases, and to the extent permitted by the applicable regulations, the center's responsibility is limited to the medical service provided in accordance with recognized professional standards.

7. Fees, cancellation and general terms

1. I agree to pay the home visit fee at the published prices and under the center's policy, and I understand that no one being at the address or a late cancellation may result in the fee being charged or not refunded under the center's policy.
2. I understand that the center may reschedule, cancel or end the visit, or decline to provide it at home, whenever the medical team judges this to be in the patient's interest or necessary for the team's safety.
3. I understand that the center may update this consent, and that the version I accepted at booking is the one that applies to this booking.
4. This consent is governed by the laws of the Kingdom of Saudi Arabia, and the competent authorities in the Kingdom have jurisdiction over any dispute arising from it.

8. Final declaration

1. I declare that I have read this consent, understood all of its terms, and had the opportunity to ask about them.
2. I acknowledge that my electronic acceptance of this consent at booking is an explicit and binding consent, and is kept with the booking record.
